Medical Preparedness Questionnaire
Making NYS Rugby Safer
NYS Rugby has instituted an aggressive campaign to assist its membership in making the sport safer. As medical preparedness and response continue to become more important in rugby event management, we are asking member clubs to assist in the effort.
NYS Rugby is asking each club to complete the following questionnaire and return by December 14. Please send to J.C. Whipple, Executive Director, at executive@nysrugby.org.

Does your club have a Medical Preparedness Plan?
If so, what plan does your club have in place?
Does your club / school have an ambulance on-site at all home games?
Is a medical professional or Certified Athletic Training (ATC) personnel supplied at practices and/or home games?
Does your club have access to an ATC for injury evaluation and/or rehabilitation?
Who supplies staffing and covers cost for ambulance (if applicable)?
What is the reporting procedure for injuries suffered during games and/or practices?
What is your club / school policy on dealing with head injuries?
If a head injury is suffered by one of your athletes, is there a policy in place prior to clearing the athlete for competition?
Is athlete medical information on file in a central location?
Does your club have its coach or any team members certified in First Aid Response?
Does your club carry a Medical First Aid Kit to all practices and games?
Is their water or sports drink accessible to both teams during home games at your site?
